According to a new report in Becker’s Hospital Review, providers should focus on addressing individual patients rather than their conditions in order to reduce hospital readmissions. In short, providers should think about patients after discharge and ensure they fully understand discharge and prescription instructions.
Hospitals around the country have implemented care transition programs in an effort to reduce readmissions. During these initiatives, the hospital sends wellness coaches to patient homes to clarify medication instructions. These initiatives have shown to reduce readmissions in some hospitals but in others, they have not significantly affected readmissions.
